This study aimed to investigate the effects of finasteride, a potent anti-estrogenic agent, on serum testosterone and serum androgenic activity and its correlation to the changes in plasma testosterone concentration after treatment with finasteride in a postmenopausal population. A total of 177 male patients (aged 66 and over) with a normal serum testosterone level (mean age, 67 years) and a normal male sexual function score (mean age, 67 years) were included in the study. The patients were randomly divided into two groups: group A (n=170) and group B (n=170) (p<0.001). Serum testosterone and testosterone metabolites, serum androgenic index (SMI) and free testosterone concentration, and free testosterone and dihydrotestosterone concentrations were evaluated. Total and free testosterone concentrations were significantly lower in the two groups (p<0.001) compared with the normal group (p<0.001). A higher level of free testosterone concentration was detected in the two groups (p<0.001) compared with the normal group (p<0.001). The serum androgenic index was significantly lower in the two groups (p<0.001), but the free testosterone concentration was not significantly different. The serum androgenic index was lower in the two groups (p<0.001). The free testosterone concentration was significantly higher in the two groups (p<0.001), but the SMI was not significantly different. The SMI of free testosterone and dihydrotestosterone concentrations were significantly lower in the two groups compared with the normal group (p<0.001).
